Online Examination Application Form for June 2012 term examination

Instructions :-

 1) Photo and signature upload is mandatory. Please get ready with scanned photo and signature seperately as JPG format.

 2) Photo and signature must be scanned from Institute Identity Card.

 3) Photo size should be : 3.5 cm x 2.5 cm and signature size should be within : 6 cm x 1.5 cm with perfect crop.

 4) Photo and signature uploaded other than your photo and signature, your application will be summarily rejected.

 5) Please complete all three step when filling up online form, take print out & submit along with DD and CCC at the related regional council/chapter.

 

 Note :-

 1) Last date of submission of examination form is 10th April 2012 hardly till 20th April 2012 subject to late fee Rs/- 300.

 2) Demand Draft shall be drawn in favour of "THE INSTITUTE OF COST ACCOUNTANTS OF INDIA" payable at KOLKATA.

 3) CCC detail for new students ( i.e. first time appearing students) will be provided shortly on the regional council websites.

 4) Other students besides new students, who have lost/not received CCC during last attempts, they need to either attach last term admit card i.e. Dec 2011 along with examination form or make the examination form verified from the related regional council/chapter for the purpose of submission of examination form
